Doptelet and Doptelet Sprinkle (avatrombopag) are prescription medications approved by the U.S. Food and Drug Administration (FDA) to treat low platelet counts in children ages 1 and older with persistent or chronic immune thrombocytopenia (ITP). Platelets are blood cells that help stop bleeding. Avatrombopag is also used to treat adults with chronic ITP for whom a previous treatment did not work well enough.

How Doptelet and Doptelet Sprinkle Work and How They’re Taken

Doptelet and Doptelet Sprinkle are both a type of medicine called a thrombopoietin receptor agonist. This means they help the body make more platelets, which help stop bleeding.

Doptelet works by acting on the thrombopoietin receptor. This helps the bone marrow make more megakaryocytes (cells that make platelets). In ITP, this helps reduce low platelet counts.

Doctors prescribe Doptelet for adults with chronic ITP, when a previous treatment did not work well enough. Doctors may also prescribe Doptelet for children 1 year and older with persistent or chronic ITP when a previous treatment did not work well enough.

Doptelet tablets are taken by mouth with food. They are used for adults with chronic ITP and for children 6 years and older with persistent or chronic ITP.

Doptelet Sprinkle is used for children ages 1 year to less than 6 years with persistent or chronic ITP. Doptelet Sprinkle comes as capsules that contain oral granules.

To give a dose, open the capsule and sprinkle all of the granules onto a small amount of soft food or liquid. Mix it and give it right away. Do not swallow the capsule whole. Do not chew or crush the granules.

Typical Dosing for ITP

The recommended dose of Doptelet and Doptelet Sprinkle for ITP depends on the person’s age and the form of Doptelet being used.

For adults with chronic ITP and children 6 years and older with persistent or chronic ITP, the usual starting dose of Doptelet tablets is 20 milligrams by mouth once daily with food.

For children ages 1 year to less than 6 years with persistent or chronic ITP, the usual starting dose of Doptelet Sprinkle is 10 milligrams by mouth once daily with food.

The doctor may change the dose based on platelet counts. The goal is not to make platelet counts normal. The goal is to use the lowest dose needed to keep platelet counts high enough to lower the risk of bleeding.

Doptelet tablets and Doptelet Sprinkle are not the same dosage form. They cannot be switched milligram for milligram. If the form is changed, the doctor will monitor platelet counts and adjust the dose as needed.

Your healthcare provider will check platelet counts before, during, and for at least four weeks after stopping Doptelet or Doptelet Sprinkle.

Doptelet and Doptelet Sprinkle Side Effects

In clinical studies of Doptelet and Doptelet Sprinkle for ITP, the most common side effects happened in between 10 percent and 31 percent of adults and children.

For adults with chronic ITP, the most common side effects included:

  • Headache
  • Fatigue
  • Bruising
  • Nosebleed
  • Upper respiratory tract infection (such as a cold)
  • Joint pain
  • Gum bleeding
  • Petechiae (tiny purple or red spots on the skin from bleeding under the skin)
  • Nasopharyngitis (cold-like symptoms, such as sore throat or stuffy nose)

For children ages 1 year and older with persistent or chronic ITP, the most common side effects included:

  • Viral infection (such as viral upper respiratory infection, COVID-19, parainfluenza, or rhinovirus)
  • Cough
  • Fever
  • Pain in the throat or mouth area

Serious Side Effects and Warnings

Doptelet and Doptelet Sprinkle can cause serious side effects that may require immediate medical attention. These include:

  • Thrombotic or thromboembolic complications — Blood clots can happen in veins or arteries and may cause symptoms like swelling or pain in a limb, chest pain, or sudden shortness of breath.

What To Know Before Taking Doptelet or Doptelet Sprinkle

Before starting Doptelet or Doptelet Sprinkle, your doctor will check your platelet count.

Before you take Doptelet or Doptelet Sprinkle, tell your doctor if you have ever had a blood clot.

Tell your doctor if you have any allergies to avatrombopag or any ingredients in Doptelet and Doptelet Sprinkle. Do not take Doptelet and Doptelet Sprinkle if you have had a serious allergic reaction to avatrombopag or any of its ingredients.

Tell your doctor about all medicines you take, including prescription drugs, over-the-counter medicines, vitamins, and herbal supplements.

If you are taking Doptelet before a scheduled medical or dental procedure and you miss a dose, contact your healthcare provider for dosing instructions.

If you are taking Doptelet for persistent or chronic ITP, and you miss a dose, take it as soon as you remember. Do not take two doses at one time to make up for a missed dose. Take your next dose at your usual time.

Doptelet and Doptelet Sprinkle are also FDA approved to treat low platelet counts in adults with chronic liver disease who are scheduled to have a medical or dental procedure.

If you are pregnant, planning to become pregnant, or breastfeeding while taking Doptelet or Doptelet Sprinkle, talk with your doctor about the risks and benefits. Doptelet may cause harm to a fetus. Do not breastfeed during treatment and for at least two weeks after your last dose.

How effective are Doptelet and Doptelet Sprinkle for ITP?

In a clinical study of adults with chronic ITP, Doptelet helped many people raise their platelet counts. Platelets are blood cells that help stop bleeding.

During six months of treatment, adults who took Doptelet had platelet counts at or above the study goal for a median of 12.4 weeks. This study goal was at least 50 × 10⁹/L. They reached this level without needing rescue therapy (extra treatment used when platelet counts are too low or bleeding risk is higher). Adults in the study who took placebo (an inactive treatment) had platelet counts at this level for a median of 0 weeks.

By day 8, 66 percent of adults taking Doptelet reached platelet counts of at least 50 × 10⁹/L. No adults taking placebo had reached that level at that time.

In a clinical study of children ages 1 year to 18 years with persistent or chronic ITP, 27.8 percent of children taking Doptelet or Doptelet Sprinkle had a durable platelet response, compared with no children taking placebo. This means they had platelet counts at or above the study goal for at least six of the last eight weeks of the study.

In the same study, 81.5 percent of children taking Doptelet or Doptelet Sprinkle had had at least two platelet checks in a row with platelet counts of at least 50 × 10⁹/L, without rescue medicine. None of the children taking placebo had this result.

How long does Doptelet or Doptelet Sprinkle take to work for ITP?

In adults taking Doptelet tablets, platelet counts started to increase within three to five days after treatment began. The peak effect happened after 10 to 13 days.

Your healthcare provider will check platelet counts during treatment and may adjust the dose based on the results. The goal is not to make platelet counts normal. The goal is to keep platelet counts high enough to help lower the risk of bleeding.

What tests or monitoring are needed with Doptelet or Doptelet Sprinkle for ITP?

Your healthcare provider will check platelet counts during treatment.

After starting Doptelet or Doptelet Sprinkle, platelet counts are checked weekly until a stable platelet count of at least 50 × 10⁹/L is reached. After that, platelet counts are checked monthly.

Platelet counts should also be checked weekly for at least four weeks after stopping Doptelet or Doptelet Sprinkle.

Doptelet tablets and Doptelet Sprinkle are not the same dosage form. If you switch between the tablet and sprinkle forms, platelet counts should be checked weekly until stable again. The dose may need to be adjusted before going back to monthly platelet checks.
